At Liberate Physician Centers we offer two different options to best suit your budget and timeline.

Option 1- (One visit)

Provide your medical records that show your diagnosis and treatment plan/medication history from your previous physicians. At that time, we will review and determine if you are a qualified patient during your consultation. Once qualified, we will create your profile in the medical marijuana use registry and provide you with your Patient ID number. You can then apply for your card with the state of Florida. When the state has issued your card, we will then provide you with your recommendation and place your order for you in the registry.

*$50 Deposit is required when scheduling to secure your appointment. Your deposit is credited toward the total cost.

Option 2- (Two visits)

First visit- Includes a consultation visit. At that time you will be required to provide us with contact information of your previous physicians so that we can request your previous medical records. Once the records are received, we will then review them to see if you are qualified based on your medical conditions. If qualified, you will have a profile created in the medical marijuana registry. You will receive your Patient ID number immediately and you will be able to apply for your card at that time.

Second visit- Upon receiving your approval or card from the state, you will have a second consult to receive your recommendation so the physician can enter your order in the registry.

Absolutely. Everything is confidential and all visits are protected under the privacy laws of HIPAA.
Anyone who is under 18 may still be eligible to receive a recommendation. A legal guardian will need to provide consent and be at the office visits.
You will be required to show proof of residency for the state of Florida. This can be a Florida drivers license, state ID, voters registration card, or a utility bill verifying your name and Florida address.
Right now due to a high volume of applicants, the state is working diligently to approve and issue cards. The timeline from start to finish is approximately 5 to 8 weeks. Unfortunately we have no way to expedite this part of the process any quicker.
